People die because the virus commands them do do certain things i.e have all your bones broken etc. The person who caused all of them to become infected was Natsuko because she was angry they didnt approve of their relationship and broke a forbidden cursed medicine thing in her mothers witchcraft storehouse. Iirc The Ousama game was actually a virus that infected all the villagers. Or maybe the original characters do show up at some point, I dunno. I think the mains should be the characters from Shuukyoku.

Honda Natsuko is Also yeah, it seems like the MAL link for the anime has the character importance wrong. Honda Chiemi is Nobuaki's girlfriend from the first manga. There is also a prequel, that is scanlated as well (but came out after the original manga).
